    BENEFITS DETAILS
    To be eligible for all insurance coverage, you must be an active, full-time field employee contracted to work for 13 weeks or more, work 36 or more hours per week (144 hours per month) and complete a waiting period of one month of continuous employment. Coverage is effective the next day following the month in which you meet the eligibility requirements. See your staffing coordinator for details on all group insurance plan options.

    401(k) Retirement Plan
    This plan is provided through Mass Mutual for employees who have worked a minimum of one month and have attained the age of 18. You are eligible to enter the Plan the first of the following month. You may contribute up to 25% of your gross pay on a pre-tax basis, greatly reducing your taxable income. In addition, the first 7% of your contributions will be matched by an MSN company contribution of 50 cents for every dollar contributed. Predecessor service counts for eligibility and vesting.

    Medical Insurance
    The Open Access plan is provided through Great West. You can choose from their Standard or Choice plans. Depending on your choice, your premium will be automatically deducted on a pre-tax basis through payroll. No provider books are necessary - access providers at www.mygreatwest.com or call (800) 663-8081. NOTE: Insurance ends 2 weeks from last day worked and can be reinstated without a new waiting period if you are rehired within 12 months and meet eligibility rules.

    Dental Insurance
    The PPO plan is provided through Guardian. Depending on your choice, your premium will be automatically deducted on a pre-tax basis through payroll. No provider books are necessary - access providers at www.guardianlife.com or call (800) 541-7846. NOTE: Insurance ends 2 weeks from last day worked and can be reinstated without a new waiting period if you are rehired within 12 months and meet eligibility rules.

    Life Insurance
    Medical Staffing Network offers free life insurance, AD&D and HIV rider benefits in the amount of $25,000 through UNUM Provident for those individuals who enroll in the medical insurance plan. Individuals who decline coverage will be provided with a $5,000 life insurance benefit at no cost to the employee. Supply your beneficiary information on the UNUM enrollment card.

    Voluntary Term Life
    Insurance This plan is offered through Unum Life Insurance Company of America. Coverage is available up to $100,000 for yourself, 50% of your coverage amount for your spouse/domestic partner up to $50,000, and up to $10,000 for your children.

    Short-Term Disability
    Our short-term disability plan covers 60% of your basic weekly earnings to a maximum of $750 per week for covered illnesses and injuries. Coverage is provided through Reliance. There is no cost to you and no enrollment forms to fill out.

    Long-Term Disability
    Our long-term disability plan covers 60% of your basic weekly earnings for the duration of a covered disability. Travel & Accident Insurance is provided through CNA in the amount of $25,000, which will cover you 24 hours a day upon eligibility. There is no cost to you and no enrollment forms to fill out. Your beneficiary information will be taken from your life insurance enrollment.

    In-Hospital Cash Plan
    If you are confined in the hospital as an in-patient, this benefit will provide a $1,000 lump-sum cash benefit for one confinement per year. Additionally, it will give you $100 per day up to 100 days per year. Dependents, spouses and domestic partners (where state mandated) can be added at tiered rates.

    AD&D and HIV Rider
    This is offered in the amount of $25,000 through UNUM. It is free to those who purchase medical coverage. Employees who do not purchase medical insurance will be covered in the amount of $5,000 at no cost to them. The policy includes a benefit for HIV exposure.

    Employee Assistance Program (EAP)
    The EAP is administered through The Allen Group. You are eligible from the first day of hire, and no enrollment forms are necessary. Assistance is available 24 hours a day, 7 days a week.
